Terms of Service
Purpose
These Terms of Service outline the conditions under which Blair Co. Testing provides mobile and on-site testing, notary, and compliance services. By scheduling, paying for, or submitting to testing, all clients agree to comply with these Terms and all related policies, including the Cancellation, Refund & No-Show Policy, Service Location Policy, and Privacy & Client Rights Policy.
1. Payment Terms
All individual services must be paid in full at booking. Payments are non-refundable once services have begun. Business and contract clients are billed per their signed Service Agreement. Late or unpaid invoices may result in service suspension and late-fee assessment.
2. Scheduling & Confirmation
All services are appointment-only. Clients must complete the Client Intake Form immediately after booking to confirm service details and request a preferred meeting location. Appointments are not finalized until Blair Co. Testing reviews and approves the intake form and location. Clients must confirm any new or adjusted location no later than 24 hours before the scheduled appointment. Failure to confirm may result in cancellation and forfeiture of payment.
3. Client Responsibilities
The individual booking the appointment is responsible for notifying all required parties of the scheduled appointment date, time, and location, including alleged fathers or additional participants. Blair Co. Testing does not coordinate attendance between parties and is not responsible for missed appointments due to non-attendance of any participant. All required parties must arrive on time and present appropriate identification as required by the testing laboratory or governing agency.
Failure of any required participant to attend the scheduled appointment may require rescheduling and may result in additional fees.
4. Liability & Insurance
Blair Co. Testing maintains general and professional liability coverage to protect both clients and company representatives in the event of an unforeseen incident, testing error, or injury occurring during services. While every precaution is taken to ensure safety and accuracy, clients acknowledge that Blair Co. Testing’s liability is limited to the cost of the specific service rendered.
5. Cancellation, Refund & No-Show Policy
Clients must cancel or reschedule at least 24 hours before the scheduled appointment time. Cancellations made within less than 24 hours or failure to appear for an appointment may result in forfeiture of payment. Refunds are not provided once services have commenced. Exceptions are made only in cases of documented emergencies and at the discretion of management.
6. Privacy & Client Rights
Client information is kept confidential and used solely for the purpose of completing testing and reporting results to authorized parties. Records are stored securely and retained for the legally required period. Clients have the right to request access to their personal information, to correct inaccuracies, and to receive a copy of test results when legally permissible.
7. Service Location, Conduct & Safety
All services must be conducted in a safe, professional, and compliant environment. Mobile services may be performed at approved business locations, community facilities, or designated meeting spaces, but not within private residences. Clients are expected to maintain respectful conduct toward staff and comply with all safety and identification requirements during the testing process.
8. Non-DOT Drug Testing (Optional Add-On Service)
Blair Co. Testing may provide non-DOT drug testing services upon request for private individuals, employers, or authorized agencies. These services are conducted in accordance with Blair Co. Testing’s internal chain-of-custody and documentation procedures.
Non-DOT testing is not regulated by the U.S. Department of Transportation and therefore follows the policies and standards established by Blair Co. Testing and/or the requesting organization. Each collection is handled with the same attention to privacy, accuracy, and integrity as federally regulated tests; however, procedures such as restroom preparation, observed collections, or use of bluing agent are performed only when specifically required by the client’s written policy or the laboratory’s instructions.
Laboratory testing fees for non-DOT collections shall be billed directly to the client or included within the quoted all-inclusive price, depending on the billing method selected at the time of service. Blair Co. Testing reserves the right to approve or decline non-DOT collection requests when the proposed collection site does not meet professional, safety, or privacy standards.
9. Record Retention & Destruction Policy
Client and testing records are retained for a minimum of two (2) years following the date of service unless otherwise required by law or a pending legal matter. At the end of the retention period, all records are securely destroyed by shredding or permanent deletion to protect client confidentiality.
10. Acknowledgment
By signing below, the Client acknowledges they have read and understood the Terms of Service and related policies of Blair Co. Testing. These Terms constitute a binding agreement governing all services rendered.
